Last time I posted a dream on here I found it extremely helpful.

Last night I dreamt that I was working on the adult general unit in the psych hospital (as I normally do) and one of the patients took this game/toy that had all of these wooden pegs sticking out of it and he started pressing the toy into his face so that all of the wooden pegs were sticking into his face. The primary psychiatrist from that unit came into the room and yelled, "Stop!" and then punched the patient in the face. The patient punched him back and then a fight started. I ran out of the room and was trying to scream, "Help! We need help here!" but my voice was so scratchy it wouldn't come out loud enough for any of the staff members to hear.

And that's all I remember.

(As far as the toy that appeared in the dream, this is not a toy that I have ever seen before).

hmm. WHEEEERRRRE'S PERNA???! My dream last night

All I can think of is, have you SI'd and not told T? Been thinking about it and wondering what he'd do if you did?

Wooden you know that would happen? My dream last night Sorry, had to get that pun in someplace.

The "scratchy" voice jumped out at me first off. Made me think of "broken record". Interesting that the patient was pressing the wooden pegs into his face and the psychiatrist "escalated" that and punched him, also in the face. Not allowed to hurt one's self but "they"/outside can hurt you. The pegs went "into" his face which is where one expresses emotions (as opposed to the psychiatrist who would be looking in your head for your thoughts). Your voice wasn't loud enough so presumably you weren't 100% into getting help? or you feel ineffectual about being a helper? "We" need help -- who? The two of them were going at it quite happily? They weren't calling for help. Was one besting the other or was it pretty "even"? Were you afraid? Usually when guys fight, all the movies :-) show a crowd gathering to watch, not to help or interfere? What were you thinking? Was the escalating "anger" getting out of control? If you're everyone in the dream, you're sticking yourself with "pins" and also punching yourself/others in the face for no reason? Any chance your T was the psychiatrist or the patient (and you the psychiatrist and just hauled off and punched him?). Sort of sounds like some of your descriptions of some of your fantasies? Too, they were being very "direct" with one another, not "playing games". The psychiatrist stopped the patient from "playing".

Well,

I think the scratchy voice is interesting. Could it be you wanted to scratch yourself? What does scratchy mean to you?

The patient with the pegs in his face, did they obscure his face from view? Could it be you were trying to disguise yourself from T seeing you? Or from Pdoc seeing you? Has this new pdoc given you a one/two punch with your new meds?

Just some thoughts.

My first thought was that the patient-psychiatrist interaction symbolized the therapy relationship to you. Maybe your own, or maybe just in general, based on all your study and experience in the internship. Your screaming and looking for help perhaps mean that you don't always agree with the therapy direction that the staff at your internship take, or perhaps that your own therapist takes with you. Also, your inability to loudly ask for help may symbolize difficulty in asking others to fill your needs. You were able to ask in this dream, but just not very effectively. The patient (a part of you) is pushing sticks into his face, which sounds like SI. A part of you appears to be asking for help with that, just not that effectively right now.

When you next go to your internship, can you look for evidence of the toy's origins?

why do you think he was doing that? boredom? frustration? showing off and / or trying to amuse the other patients and / or you? do you think it was hurting him or not?
