Not sure where to start my "emergency session" with T. I've never had one before. I've gone inpatient without calling, but I've never booked a session ahead of time in order to avoid worse outcomes. I don't how to bring things up; because, almost everything is quite, literally, going wrong. I'm very upset in general. Things from the past have blown up in my face. Work is awful. I got sent home early. Because I suspect, I was acting paranoid. My family situation is starting to go down hill, and I don't know how much more I can take, at this point.

I find it often helps me to write down topics I want to talk about before a session. I may not stick to the list, but it helps organize my thoughts, and then I'm less likely to forget something important. I've had a couple "emergency sessions" with a T before, including one fairly recently. Not sure if this is going on in your head at all, but I was worried that, once I saw T, he'd be like, "I'm sorry, why did you need this extra session?" because maybe I wouldn't have seemed upset enough. But he said he wouldn't think that. (A previous one I had with ex-T, I didn't have that concern because I was clearly a wreck, and we both knew it.)
Hope it goes well and is helpful... And honestly, you could just start by saying what you said here, how it feels like everything is going wrong... |

I agree with LT, making a list might be helpful (you've started one here in this post!) and you don't have to memorize it or anything but it could be a helpful reminder.
I have had one emergency session with my T regarding a traumatic situation I was in (she actually called ME, she had heard about the situation and called to ask if I wanted an appointment) so when I went in that's pretty much all we talked about.
