I once had a dream that I was boarding an elevator on the 20th floor of a 20 story building. It started down and started shaking a lot. It then it stopped on the 19th floor to let more people on. I had a very strong urge to get off so I did. The elevator doors closed and a few seconds later I hear a loud explosion and the sound of screaming and the elevator falling, eventually slamming onto the bottom floor resulting in a second explosion, killing everyone on board. I saw lots of smoke and flames were coming up the elevator shaft. The last thing I remember were the fire alarms going off, people evacuating the building and the sound of fire trucks approaching. After that I had a birds eye glimpse of what happened in the elevator, apparently everyone burned while the elevator fell and killed them all on impact. Creepy and weird dream.

Buildings typically represent the physical body in dreams. That you were in the top floor shows that your awareness starts intellectually, in your brain. It's also where things start to go wrong. Well, sort of. It isn't until the elevator starts to go down that you follow your intuition, and by golly you were right! Something just below your neck is going wrong, and destroying the rest of you in the process. I'd almost want to call it the face...which would mean that it has to do with how you view and interact with the rest of the world. But in following your intuition, you gain a higher perspective, reflected in the bird's eye view.
I also have to agree with Mike - what is this significance with the number 20? Only you can answer that.

Fire is Transformation. Those people (things and feelings, in real life) in the elevator are now out of your life.
