I have downloaded an app that I like but find insufficiently challenging. I have been using it for a few days. It has me tensing and relaxing for 3 seconds and then quickly alternating between tensing and relaxing every 1 seconds. I like it, but my pelvic floor PT assigned the exercise of tensing for 10 seconds. Are there apps that include longer tensing exercises? Or apps where I can customize the duration of the hold? What is your favorite Kegel app?

Just saw this. I use Squeezy which is an NHS endorsed app, it sounds similar to yours, Each set comprises of 10 x10 second hold followed by relax, then 10 short squeezes. Those are 3x a day. You can alter the duration on the app but those are the recommended.
Thanks for this!

I beg forgiveness, but may I gently suggest you try a breathing app? Consider that the contraction can be done on the inhalation and release on the exhalation. Or vice-versa.

One that is free and customizable (and offers English, German, French and Nederland) is called simply "Breathe." It's by Havabee, "Made with in India."

While it offers several popular breathing protocols, the user can set up custom protocols. It does have a "hold" command (no breath), that can be made very short so as to virtually eliminate it.

Apple: Breathe: relax & focus on the App Store
Android: Breathe: relax & focus - Apps on Google Play

I wanted to add, I’ve been using Squeezy a year now and I’ve definitely had an improvement.
