It's so hard not knowing from one day to the next if i'm going to be ok or am i going to do or say something wrong do to the meds. I hope i get an snwser from someone else so i know it's just not me.......

Can you talk to your pdoc about this? You may need a change, or an upped dose, or more doses during the day.
Do you have any physical health problems? Such as Crohn's or anything that makes it hard for your body to absorb things before ridding it out of the system?
Is there any common things (Situations, meds, and such) that happen or don't happen on the day that this happens? do you get nauseated or have a stomach bug?

I have never been officially diagnosed ADHD, but I have strong traits.... and i experience variations of quality (i have some other MD issues, but I don't think that is the problem here).

questions is how big are the differences. I think even "normals" have better days, not so good days and straight godawful days.

Maybe it depends on what are you doing? Something is more attention capturing? Stress can be influence. Even the weather and biorythms (some are more sensitive to it than others).

are you seeing a therapist? what is your biggest problem? Lack of focus, disorganization, lack of motivation, perhaps underlying anxiety issues?

I would keep a journal for awhile; see if you can see any trends around nutrition or other physical aspects (sleep quality) and when you have better or worse days?

Quote:
Urinary and stomach pH levels can have a strong effect on DL-amphetamine excretion and absorption. An acidic stomach and GI pH will decrease the absorption of Adderall, and acidic urine levels will decrease the reabsorption of the drug through the renal system. Co-administration of acidic substances (e.g., citric acid) causes decreased renal reabsorption of DL-amphetamine; whereas, alkaline agents (e.g., antacids) may cause a marked increase in renal tubular reabsorption. The increased reabsorption can increase the retention of amphetamines, with potential to result in dangerously high serum levels.

I've had issues with ADHD meds wearing off. Usually it meant I needed a higher dose, or a second dose later in the day.

Edit: also, yes, they pretty much work every day. Some days more effectively than others, but I can definitely tell the difference from not taking it at all.

I've noticed this too. I've been on 20mg XR twice daily for years. I took name brand Adderall until about 3 years ago. Usually it's fine but I occasionally run into a pill that seems like it's a placebo or just diluted. There are a few different makers of generics, one time my pharmacy had to give me mixed bottle of two different makers and I noticed some slight differences but I can't rightly recall the specifics. I figured my mind was playing tricks on my so I put it out of my head.

Quoting from wikipedia on generic drugs, "The FDA requires the bioequivalence of the generic product to be between 80% and 125% of that of the innovator product.".

Adderall is made of 4 different amphetamine salts. From what I've felt, it seems like the generics have slightly different ratios of those four salts. This is pure speculation, I really don't know for sure, but they feel slightly different in very subtle ways and that's a reasonable, though currently unsubstantiated, explanation.

I take amphetamine salts and I can say I definitely relate. There are days (like today) where I think it isn't working.

I have the, kinda, the same problem with my generic Adderall (Barr/Teva Manufacturer). I was taking 10mg twice a day for about a year and ever had a problem with the effectiveness. I was increased to 15mg twice a day and noticed that it does not work nearly as well...if at all. There are times I wonder if it is a placebo pill or if there is some problem with the amounts in each pill during manufacturering. Sometimes I can tell very little that I took it and other times, it feels like I took nothing at all. I also hate the fact that I feel very tired on the 15mg...almost like I have a hangover most of the time. I went back to the 10mg for a few months and they were effective but not enough of a dose after taking it for a little over a year. So, thinking maybe it was me, I asked the doc to try the 15mg again, which I am taking right now. I HATE IT!!!!!! It doesn't work and I've tried finding another generic brand, such as Sandoze (not sure if spelled right) but could not find it at any of the pharmacies near me and the brand name was on back order.

Well after giving the 15mg a second try, I am going to try to maybe ask to have some combo of 10mg ie: 10mg 3 X/day, or 10mg 2 tabs in am and 1 tab in afternoon
I'm not sure how my insurance will feel about that but I know that the 10mg works and the 15mg does not. The odd thing is that they are both from the same company. I am completely sure that there is a difference in the two just don't understand how the lesser dose works better and why. I plan to write the company...not saying that they care but no harm in voicing my concerns.

To answer your question, I have noticed a difference in effectiveness of dose and there may also be an issue with your 20mg. You may want to try for the brand name or a different generic. I would also ask for another dose in the afternoon if I were you. You could always try and go with the 10mg twice a day, which may work for you as I know it did for me.

Good luck!

On Feb 13th I was diagnosed, again, with ADD. This time I decided to try meds. Vyvanse was too expensive so that wasn't an option. I was prescribed generic Adderall (d-amphetamine salt combo/MFG Core Pharma) 20mg twice daily. At first it seemed like something was working but then after a week I felt like I was taking a sugar pill. It was unreal. I even took 2 at once and it just made my heart race but then nothing...

Then I started taking it and FALLING ASLEEP. What? Seriously. This afternoon, just to test the theory and because I need to get more Ritalin, 3rd drug in the trial, I took 60mg of this generic Adderall and FELL ASLEEP. What the hell...

So now my psych gave me 4 scripts for Ritalin that are a week long each. He told me to try different pharmacies for different drug manufacturers. It's weird how all that works...I didn't know there was such a difference in name brand and manufactorer.

So brand name Adderall...that works better for some? I had to buy the name brand Vyvanse and it worked but was so expensive so I'm thinking that Adderall may work for me if it's REAL Adderall. I don't know. But from what I'm hearing you guys say, there is a noticeable difference in brand name ADD meds and generics.

This is a common complaint about the generic produced by Barr or Teva (both are owned by the same company). These companies have a bad track record for many of the generics they produce. I have personally experienced the difference between their version of Concerta, and the brand name (I'm in Canada where Concerta is off patent). I had worked up to 54mg/day and it always like rolling the dice as to what effect I would get. Sometime I would feel nothing, other times I was extremely stimulated, unfocused, and just a mess. I switched to brand name and found I only needed 18mg/day and the drug performed like a time piece, I always knew what to expect.

If you're going to use generic, you have to avoid certain companies like Teva.
