Interesting update on this. For kicks I searched for such a program and there are two in Minnesota (where I happen to live). One of them is at the Mayo clinic in Rochester. It's an 8 day inpatient rehab program for smokers. They're boasting a high success rate and while I'm not loving the idea of being locked in rehab, this could be exactly what I need. I've filled out the preliminary paper work to see if my insurance will share the bill (the only way I could afford to go to the five thousand dollar program).
