I'm hoping to convince my son to go inpatient, but I know that the no-smoking policy at the local psychiatric hospitals will discourage him from considering it. I'm wondering if anyone knows of a great treatment center that allows smoking. He has a slew of anxiety disorders and OCD for the past 7 years, and recently developed paranoia within the past few months.

You don't mention where you are, so it is hard to know where to suggest. Most don't allow smoking anymore due to city ordinances against smoking. I know the hospital I use once allowed smoking (which personally I hated because I don't smoke and sometimes the rooms would smell of smoke because peoples' clothing reeked of smoke and left the smell wherever they went). However, the city added a new ordinance against smoking. The hospital tried to get a waiver for the psychiatric unit, but it was a no-go. The do provide patches and ecigarettes (I think that is what they are called; I'm clueless) as an alternative.

Sometimes voluntary patients can have privileges to go smoke outside.

I doesn't matter where we are. I'll take him anywhere for him to have the best shot at getting better. It doesn't have to allow smoking inside. Just as long as it allows smoking breaks or a smoking area. I believe most psychiatric hospitals are getting away from allowing smoking, but there HAS to be some that allow it on some level. I know he won't go inpatient if he can't smoke. Patches and gum won't be reasonable in his mind. And why make treatment harder on him? He needs mental health treatment more than he needs quit smoking treatment.

I was at one where they gave the smokers "something" and fake cigarettes.
No need to smoke because what-ever they were given provided the nicotine fix and the fake cigarette handled the oral fixation issue.

There were lots of people that had them. I would call your local hospitals and ask how they handle smokers.

Have him try full strength e cigarettes. You buy the filters by the brand and strength you usually smoke. They're heavier then normal cigarettes if he's okay with it. Each filter has about 2 packs in them.
