I get intrusive thoughts a lot, too. Most of it is suicidal or self-harming ideation. I've never self-harmed, so I'm not sure why I get that. I have felt suicidal, but mostly these intrusive thoughts happen outside of that state. Sometimes it happens when I'm either hypo (anxious & agitated) or depressive, but sometimes it just happens when I'm totally calm and it's out of the blue.

Even though my thoughts are mostly self-directed, I have had them directed at others, too, including my husband and kids, acquaintances and random strangers. I know I wouldn't act on those thoughts, but they are definitely disturbing regardless.

ETA: The thoughts I've had directed towards my kids were primarily when I was suffering from PPD. I had it bad with both, lasting for years with each of them. It sucked big time.
