I don't have a car and frustrated it will take me 6hrs of travel time round trip for a 1hr appt. I don't have any friends or family and have pretty profound social anxiety and agoraphobia so the thought of being away from home that long is terrifying and in a area im unfamiliar with

Try some grounding techniques, breathing exercises, maybe that would help. Why 6 hrs.? How far away is it?

What has me scared is being in a familiar area and will cause me to dissociate. I will google grounding techniques. The buses are *** backwards here 3 hrs each way. Think the place is like 20miles away

Wow. That's a long trip for a place that's 20 miles away. Have you looked at other possibilities for travel to there other than buses? Maybe the therapist has some ideas on a better way to get there and back?

Is there any other options for transportation? Have you discussed it with your therapist? I think some breathing exercises would certainly help with anxiety, though you need to start practicing them now and get yourself comfortable with them, I use the 4 sec inhale, 4 sec. hold, 4 sec. exhale
