Not sure if this is the right forum for this question but, was wondering how I would go about getting a copy of my records? Has anyone here done this? What did you do?

You are entitled to a copy of all your medical history. What you need to do is go to the places where you were treated and simply ask for a copy of your medical records.
Sometimes these records are kept away from the facility in another place and you may have to pay a fee to get copies made and these records be sent to you. However that is not always the case. And you are asked for proof of ID and to sign a form and then you are given your records.

Thanks. I called them today and the medical records person talked to me. She seemed in kind of hurry, but basically told me I didn't need any appointment. I just walk in anytime and tell the front desk I need a Review&Request Records.

Then someone will come out and take care of me. And that it would cost me .75cents page, I think. Since I was being taken care of for a couple of years at this particular place it may be a steep bill. But it's important that I have it.

Anyway, thanks again for the help and response.

I have a problem avoidance method I use. it is called M.M. >something<
in M.M. , call and speak to another person, see if you get the
same information, if you are told different, or that you will need some
id, like birth certificate or S.S. I. card to access these records, then
having made the call will save you a wasted trip.

Can't help you with that im afraid - perhaps just bring it up with your CPN/Psych when u see them? I always used to joke that because i was passed between so many doctors and would subsequently have my notes lost, it would get to the stage where i considered writing my own notes and taking them with me to each session lol
