Hi,

I am new or not so new on this. I had a manic episode while a teen, many depressive ones, and also mixed ones, but during the mixed ones I thought it was just stress or CPTSD trigger that happened to often.

On Friday my doctor confirmed they were dysphoric mania. He said he thought valporate or carbamazepine, but he didn't thought about lithium that much.

I see him on Tuesday.

What have been your experiences with these medication and what information you can provide me?

Thank you, I am still confused, I never thought I was really bipolar/schizoaffective.

Valproate ends mania effectively, but can make you amazingly hungry, as well as a few other side effects I didn't get. The hunger effect seems to be wearing off now for me at about 4 months at therapeutic level. Losing the weight I gained will be challenging, but an acceptable price for what I gained.

I just started lithium and am in a mixed episode now. I have bipolar/schizoaffective, as well.

I hope they can figure out the best med for you. Good luck!

Thank you both,

I am afraid of valporate since I have POS and it makes me naturally to gain more weight, and it is not for looking pretty or not, high weight with POS is a bomb of symptoms, it makes POS worse, and also, being physically slow by overweight, getting tired easily, possible sleep apnea... Will likely make me depressed and insomniac, so I would need lamictal and extra pill for sleep.
While lithium seems more weight neutral I am afraid of ''getting dull'' on it as many people describe it.

I see carbamazapine more benign ... but I don't have enough information.

I don't have experience with any except lithium and I wasn't able to get to therapudic dosage as I experienced side effects. Why does your doctor only provide these options when there are many others to consider? I'm sorry to hear about your health issues and I know from experience how health issues can complicate the medicine options. Good luck on your decision and I hope you get the answers you seek here.

I can't take antipsychotics, SSRI, SNRI, trycycles (i don't know how to spell it), buspirone is not available in my country.

Benzodiacepines do nothing for me at a therapeutic dosage, I have to take mega ones which is dangerous.

Gabapentin absorbs really different depending on many factors, from 30% to 60% of it. So I was put on pregabalin that kills my anxiety and depression.
He added trazodone on Friday for the agitation-mixed episode. He said he cannot give a mood stabilizer without being able to monitorize the first days that would be weekend.

Which other options I have that are not mood stabilizers?

SSRI's don't work for my daughter, and SNRI's are somewhat ineffective too. My daughter is on a SNRI (Serzone) and a Trycylic (also unsure of spelling) Desipramine, along with Tegratol as a mood stabilizer. She was put on Lamictal when IP, but they were too aggressive with dosage increases and she got the rash, so she was taken off that and put on the Tegratol instead.

My current doctor says that for mixed states Lithium is the best of those three, but he also said that current research has shown Lithium plus Valproate is even more effective. I started Valproate (with the Lithium I have been on for years) to try to calm me down but stopped the Valproate a week later because of the weight gain issues. In the end an AP Haldol, brought me back to stable.

lithium worked for me. they raised me pretty quick inpatient to therapeutic. but I think being bombed with seroquel with the lithium was the combo for me

So adding lithium or carbamazepina (tegretol) might be the best...

I tried carbamazepine during a trip to New York in the late nineties and it transformed me into something I am literally unable to describe, so I am neither able to recommend nor endorse it.

I was diagnosed bipolar un November last year, after being like 12 years on antidepressants. They last manic episode started on June last year. I was put on Depakote and lithium and Seroquel, and stopped taking they antidepressants. It helped but I entered a mixed episode. At least for me, the miracoulus drug is Zyprexa: it kills manía un a few days. It comes with huge weight gain and sedation.

I was put on Depakote and Lithium and they did their work, but not fully. Then came Zyprexa: it killed the episode in less than a week.
